| ..... | How to sign up for a class ALL 2012 CLASSES ARE SOLD OUT
1) Print and complete the 4 forms found at the bottom of this page. Important note on the medical form: If you answer 'yes' to ANY of the medical questions, your physician must complete page 2. If you answer 'no' to all of the questions, send only page 1 of the medical (physician release not required).
2) Remit with a deposit of $75 per student to: Ohio Scuba, 1851 Pathfinder Ct, Xenia, OH 45385. You can also use Paypal for the deposit (david@electronix.com).
3) After receipt of your deposit and forms, your courseware and class information will be sent to you via Priority Mail. Please note, reservations are on a first come, first deposit received basis. We have very small class sizes, and sell out quickly. Please sign up early!
